Immunizations
To enter the seventh grade your child must be up-to-date on their immunizations. They must have:
MMR- 2 shots
Varicella- 2 shots
Hepatitis B- 3 shots
Polio 4 shots
Diphtheria and tetanus toxoids and acellular pertussis (DTaP) – 5 shots
In addition, 1 shot of
Tetanus and diphtheria toxoids and acellular pertussis (Tdap) vaccine
Please send in your child’s physical with their immunizations before the start of the seventh grade.
